Acute-on-Chronic Liver Failure (ACLF): What It Is, How We Grade It, and What Treatment Actually Looks Like
ACLF is the condition where a relatively stable liver — damaged by years of hepatitis, alcohol, or metabolic disease — suddenly collapses in response to a new insult. It is the most dangerous presentation in hepatology. Survival depends on how quickly the triggering event is identified and managed, and on whether organ support reaches the patient before irreversible failure sets in. The APASL AARC definition — developed specifically for Asian populations — differs from the European EASL-CLIF criteria; both identify a high short-term mortality risk, typically within 28 days. What Is ACLF — and Why Does the Definition Matter?
The term ACLF was formalised by the Asia Pacific Association for the Study of the Liver (APASL) in 2009 and significantly refined through subsequent consensus statements, most recently the Kyoto Consensus in 2021. The APASL defines ACLF as an acute hepatic insult manifesting as jaundice (serum bilirubin ≥5 mg/dL) and coagulopathy (INR ≥1.5), complicated within four weeks by clinical ascites and/or encephalopathy, in a patient with previously diagnosed or undiagnosed chronic liver disease.
The parallel European definition (EASL-CLIF consortium, the CANONIC study) is broader: it includes patients with acute decompensation without a liver-specific precipitant and grades ACLF purely on the number of organ failures. Both definitions capture a high-mortality syndrome, but they identify different patient populations. The APASL definition, built on Asian cohort data, places the hepatic insult at the centre — a distinction that matters in India, where HBV flares, TB drug toxicity, and Ayurvedic/herbal hepatotoxicity are common triggers.
Practically: if your patient has known chronic liver disease, presents with deepening jaundice and a rising INR, and develops ascites or encephalopathy over days to weeks — that is ACLF until proven otherwise.
What Triggers ACLF in India?
Bacterial Infection
The commonest precipitant in Indian cohorts — spontaneous bacterial peritonitis, pneumonia, UTI, and skin infections. Sepsis drives a cytokine storm that overwhelms an already-compromised liver.
HBV Reactivation
Hepatitis B flares — whether spontaneous or triggered by immunosuppression — are a major cause of ACLF in Asia. Rapid antiviral therapy (tenofovir or entecavir) is mandatory.
Alcohol Binge
A heavy drinking episode superimposed on alcohol-related cirrhosis is the prototype ACLF trigger. Acute alcoholic hepatitis adds its own inflammatory burden.
Drug and Herbal Hepatotoxicity
Anti-TB therapy (INH/RMP/PZA) and Ayurvedic preparations are common culprits in India. A patient on TB treatment with rising bilirubin must be evaluated for DILI-triggered ACLF — stop the drug immediately.
Portal Hypertension Events
Variceal bleeding or large-volume paracentesis without albumin replacement can precipitate acute hepatic deterioration in a cirrhotic.
Surgery and Procedures
Elective surgery in a patient with unrecognised chronic liver disease, or any procedure imposing ischaemic stress, is an under-recognised trigger.
Grading ACLF: The AARC Score and Organ Failure Count
The APASL AARC working group developed the AARC-ACLF score based on six variables: bilirubin, creatinine, INR, lactate, sodium, and hepatic encephalopathy grade. A higher score predicts higher 28-day and 90-day mortality. Many centres also use organ failure count to assign grade (ACLF-1 through ACLF-3) because it maps intuitively to transplant urgency.
ACLF-3 — three or more organ failures simultaneously — carries a 28-day mortality exceeding 70–80% without transplant in most published series. The trajectory over the first 48–72 hours matters as much as the admission score: a patient who deteriorates despite maximal medical therapy needs transplant listing without delay.
Treatment of ACLF: What Can Be Done, and What Cannot
There is no pharmacological cure for ACLF. Treatment runs on three parallel tracks simultaneously.
Track 1: Remove the Trigger
Treat the precipitant aggressively. Antibiotics for bacterial infection, antivirals for HBV flare, cessation of the offending drug for DILI, haemostasis for variceal bleeding. Removing the trigger is the only intervention with a clear mechanistic rationale.
Track 2: Organ Support in ICU
Renal replacement therapy, mechanical ventilation, vasopressors for hepatorenal syndrome. Albumin infusion for circulatory dysfunction is evidence-backed. MARS and plasma exchange may bridge to transplant in selected cases — not as a substitute for it.
Track 3: Nutrition
Aggressive early enteral nutrition. Patients with ACLF are hypermetabolic and hypercatabolic. Protein targets of 1.2–1.5 g/kg/day are appropriate. Sarcopenia is an independent predictor of both medical and post-transplant outcomes.
When Is Transplant the Right Answer in ACLF?
Transplant in ACLF is a time-sensitive decision, not a last resort. Listing should be considered early for ACLF-2 or ACLF-3 patients, or any ACLF-1 patient whose score is deteriorating despite 48–72 hours of optimal therapy. Contraindications must be assessed in parallel with listing. The patient “too sick” for transplant today may be unsalvageable tomorrow.
Living donor liver transplant (LDLT) — the dominant modality in India — has been performed successfully in ACLF, including in patients on vasopressors at the time of surgery in experienced centres.
Why the APASL AARC Definition Matters for Indian Patients
The APASL AARC working group has produced the most comprehensive prospective ACLF data from Asia, including Indian patient subgroups. The criteria, scoring, and treatment algorithms are designed for Indian hepatology units: patients with undertreated viral hepatitis, significant nutritional depletion, high rates of TB and herbal hepatotoxicity, and limited organ support access in tier-2 cities.

Frequently Asked Questions — ACLF
Is ACLF the same as acute liver failure (ALF)?
No. Acute liver failure occurs in a patient with no prior liver disease; ACLF occurs in a patient who already has chronic liver disease. The causes, treatment protocols, and transplant criteria differ. Both can require urgent transplant but the evaluation pathway is distinct.
My family member is in the ICU with ACLF — what should I ask the team?
Three questions matter most: (1) What is the AARC score or ACLF grade, and is it improving or deteriorating? (2) Has the precipitating cause been identified and treated? (3) Has a transplant specialist reviewed the case? For ACLF-2 or ACLF-3, requesting a second opinion from a dedicated transplant centre is appropriate and does not need to wait for stabilisation.
Can ACLF reverse without a transplant?
Yes — in ACLF-1 and some ACLF-2 patients where the trigger is effectively removed. The trajectory in the first 48–72 hours is the best guide. A score that rises despite treatment is a signal to accelerate transplant workup.
What is the role of plasma exchange or MARS in ACLF?
Their evidence base supports use as a bridge to transplant — buying time while transplant logistics are arranged — not as a definitive treatment. They do not regenerate a failing liver.
